MS GradPath Forms

In order to graduate, students must submit GradPath forms in UAccess:

Form Submission Deadline
Responsible Conduct of Research Statement End of the first semester in program
Transfer Credit Form (only if using external transfer courses) End of the second semester in program
Plan of Study End of the second semester in program
Master’s/Specialist Committee Appointment As soon as the Plan of Study has been approved. This form is required whether or not you formed a thesis committee.  Non-thesis students should select the Director of Graduate Studies as your major advisor.
Master’s/Specialist Completion Confirmation Submitted by Graduate Program Coordinator once final grades are posted and degree requirements are met.
